CORE provides rapid-response stabilization and diversion services designed to prevent individuals from entering unsheltered homelessness and to support those facing immediate housing instability. Our services focus on early intervention, coordination, and connection to safe alternatives before a crisis becomes chronic homelessness.

What We Offer

  • Because shelter beds and affordable housing are extremely limited in many communities, CORE emphasizes flexible stabilization strategies that do not depend solely on traditional housing availability. 

    Diversion and stabilization services may include:

    • Housing problem-solving and stabilization planning

    • Reconnection with family or trusted community supports

    • Coordination with hospitals, detox programs, and other institutions to prevent discharge-to-street

    • Referrals to shelters (including DV shelters), housing programs, or social services

    • Short-term housing assistance when available

    The goal is to prevent individuals from entering unsheltered homelessness and support safe transitions following moments of crisis.

  • CORE serves as a bridge between individuals in crisis and the community's network of services. We collaborate with healthcare providers, outreach teams, shelters, and community organizations to ensure individuals are quickly connected to the right resources.

    Support may include:

    • Referrals to mental health or substance use services

    • Connection to case management programs

    • Coordination with local service providers

    • Coordination with community resources to attain formal education for non-traditional students.

    By strengthening coordination between organizations, CORE helps improve access to services and support long-term stability.

  • Extreme weather conditions create significant health risks for individuals experiencing unsheltered homelessness. CORE activates targeted outreach and stabilization efforts during dangerous fire events to help and protect vulnerable individuals.

    Our weather and fire response efforts may include:

    • Outreach to individuals known to be unsheltered

    • Coordination of safe indoor placement options

    • Distribution of emergency/hygiene supplies

    • Connection to warming or cooling locations

    • Rapid secure placement including motel/hotel or shelter

    These efforts focus on protecting life and reducing exposure-related harm during severe weather conditions or building fires.

Once a referral is submitted, our team review it and may reach out to obtain more information.

Connect with Community

We contact our community partners to assess the availability of services and resources, then plan a coordinated response.

Conduct Outreach

Once services are secured, we contact each individual, meet with them, and conduct a comprehensive assessment in their own environment.

Plan with Purpose

Following the assessment, we put the plan into action and provide ongoing services through stabilization.
